Help for lost PPO by Lt Col Uday Naik

Dear Sir,

I retired from Corps of Engrs in 1998. I am settled in Delhi and now facing a unique problem. I was drawing my pension from SBI Main Br, Sansad Marg, New Delhi. Due to their treatment to the customers, I decided to transfer my pension to NOIDA and submitted all documents accordingly to them. These incl my ORIGINAL PPO. Unfortumnately they have lost the ORIGINAL PPO. Now they are willing to issue a letter stating that they have lost the ORIGINAL PPO.

Can somebody let me know the exact procedure to ontain the duplicate PPO?
